Throwback American airlines sends a non-ETOPS A321 to Honolulu
On august 31 2015 12 days after american airlines started using the a321 on the LAX-HNL route they flew a non-ETOPS a321 to HNL and they realized midflight when a crew member found out that they didn't have the equipment needed for the ETOPS certification. This happened 10 years ago due to a scheduling error when some A321 aircraft in AA's fleet did not yet have ETOPS 180 rating. Hardly news.
The report stated 2015 - a lovely "historical document" in "Galaxy Quest" terminology..
given its painted on the nose gear doors, MANY people missed this error. Normalization of deviance?
