Interesting. We always on being cleared for takeoff, turned on the landing lights and away we'd go. Line & Wait or taxi into position & hold clearances would see only the strobes come on. Takeoff clearances were always confirmed by just looking at the landing light switch(s) position & CRM. Didn’t matter if we were in a L-1011, Gulfstream or a Cherokee.

" line up and wait" is the "taxi into position and hold" for the old timers and SA, let alone CRM, seems a foreign concept for this Happy Hawker crew.

I know they have to be thorough but 2 years to investigate seems excessive.

ATC if you listen to the Audio, cleared the aircraft to line up and wait, then got focused on two other aircraft, and forgot about him.

It’s ATC who should be given a number to call, they have a problem.

Line up and wait means line up and wait. Not line up and when you get bored take off.

It doesn't matter whether ATC did or did not forget about the Hawker.

If the Hawker pilot felt he was forgotten about, he could have contacted ATC. The one thing he couldn't do was say "Screw it, we're outa here."

I do believe ATC lost track of him or they might have noticed that he was rolling and maybe could have stopped him.

Yes they do - it is being short-staffed. But that's not a reason for them to call, nor a reason for ANY pilot to ignore the last order given by ATC.
